Colour is converted to the profile the printer runs, so the proof you approve matches what comes off the press rather than what the screen showed.
Inserts, flyers and multi-page publications are imposed to the sheet and the fold the job will actually use, with crop and registration marks in place.
Soft proofs for layout and text, and a printed proof when colour matters, each one signed off before the file is released.
Final output in the format your printer asks for, labelled with the job details so a reprint comes out the same as the first run.

Yes. You get a proof to check text, layout and colour, and nothing goes to the printer until you have signed that proof off.
Yes. Tell us which printer and which press the job runs on, and the file is set up to their specification: bleed, colour profile, imposition and output format.
It depends on the state of the file. A clean, print-ready PDF moves quickly; artwork that needs fixing or rebuilding gets a date agreed with you before any work starts.
